| A 501(c)(3) CHARITABLE ORGANIZATION FOR THE BENEFIT OF EQUINES IN NEED |
Criteria ~ Must be a 501(c)(3) equine rescue/retirement/rehabilitation/adoption shelter, or have permission to utilize another organization's 501(c)(3) status as fiscal agent; ~Must be in existence and operating as a 501(c)(3) for at least one full year ~ Programs considered may include, but are not limited to, the rescue, care, and rehabilitation of injured, old, abused, starved, slaughter-bound, and neglected horses, as well as those horses used for medical experimentation (including Premarin mares and foals), and permanent care for aged equines living out their final years; ~ Qualifying organizations must provide 1) a permanent sanctuary in which rescued and aged equines may live out their last years in peace and find a dignified retirement and/or 2) a rehabilitation and proven adoption program that serves in the best interest of the equines that are placed. Details of the adoption program requirements must be submitted at the time of proposal. Good To Know ~ Brennan Equine Welfare Fund will show preference to well run, financially responsible, all-or mostly all- volunteer based facilities ~No email or FAX applications/information will be accepted. Only snail-mailed, hard copies will be accepted ~Grants approved are generally from the few $100's to $5,000 ~All grant applications and enclosures must be complete. No calls, emails or other contact will be made to request missing items or information. If your grant application packet is missing information, required signatures or enclosures, it will be considered incomplete and therefore disqualified for consideration. ~All submittals, photos, and materials become property of Brennan Equine Welfare Fund Required Proposal Information for 2012 The following is required for consideration; (please make sure all items are included in your grant request packet. Incomplete grant requests will be disqualified.) 1. Completed Grant Application 2. Profile of Organization: a) Brief summary of organization's history and statement of organization's mission. b) Brief description of organization's current projects/programs and activities c) If there is an equine adoption program in place, please submit a copy of the adoption program requirements, as well as a copy of the application for adoption/adoption contract (please note that the applying organization's adoption agreement must be inclusive of a strict no breeding policy for consideration of grant funding) 3. Statement of Need: a) Statement of need the project is attempting to meet and evidence of that need. 4. Program/Project Description: a) Summary description of overall program/project to be funded under this grant b) Brief description of how you plan to use the grant funds c) Timetable for implementation 5. Required Financial Attachments: a) Organization's IRS tax-exempt 501(c)(3) determination letter b) If using another organization as fiscal agent, please include a letter of authorization from that organization, along with a copy their 501(c)(3) determination letter. c) Organization's year-to-date income/expense (P & L) report d) Organization's income/expense (P & L) report for the most recently completed fiscal year e) Organization's most recent annual report/independent auditor/CPA report (if available)* f) Organization's most recent IRS Form 990* *Note: If submitting IRS Form 990-N Postcard, you MUST enclose item "e" 6. Veterinarian Reference Form a) Please have organization's attending Veterinarian print out and complete the Veterinarian Reference Form. (Must be filled out in its entirety by attending Veterinarian. Obvious discrepancies will disqualify application) 7. No fewer than six (6) photos of the facility, including: 1) pasture/paddocks 2) fencing 3) barn 4) stalls 5) water sources 6) some current residents 8.
